Monitoring Referral Efficacy and Outcomes (10 Contact Hours)

Overview
Curriculum

Monitoring Referral Efficacy and Outcomes is a 10-hour continuing education course for Certified Mental Health Professionals (CMHPs) who connect people with services and want to know whether those connections actually work. A referral is only successful when the person reaches the service, engages with it, and has their need met. This course teaches CMHPs to replace open-loop referrals with closed-loop referrals, define and track referral outcomes, understand why referrals fail, measure outcomes that matter to the person, and use simple continuous quality improvement methods, including the Model for Improvement, Plan-Do-Study-Act cycles, root cause analysis, and run charts, to improve referral practices and partner networks, all while protecting privacy and staying within the CMHP role.
