Reviewing and Updating the Service Plan is a 10-hour continuing education course for Certified Mental Health Professionals (CMHPs) who help keep service plans accurate, current, and useful over time. A service plan written at intake describes a person at one moment. People change, circumstances change, and what helps changes. Plans that are not reviewed drift away from the person's real goals, fall out of compliance with funding requirements, and stop guiding services. This course teaches when and why plans must be reviewed, how to prepare the person and the data, how to measure and summarize progress, how to bring the person's voice into the review, how to decide whether objectives should continue, change, or end, how to recognize changes in circumstances that call for clinical review, how to coordinate with the team, how to document reviews, and how to plan transitions and discharge. It completes Block 2 of the CMHP program.
